Join for one annual fee and enjoy preventive care, including exams, cleanings, and X-rays,
and 25% discounts on conventional dental treatment.
Plan Details
Membership:
Individual Membership is attached to one patient,
Couple Membership is attached to two specified people living within the same household,
Family Membership is attached to two adults and two legal dependents (children up to the age of 26) living in the same household.Covered Benefit:
100% coverage: two exams, two sets of x-rays, two cleanings
Frequency limit: after two times of 100% coverage, Standard fee schedule applies.
25% off all procedures: all conventional dental treatment will be provided at a 25% discounted fee from the Standard fee schedule.
(exceptions apply to non-conventional, optional procedures, and procedures with frequency limit)
